Registration Requirements

Proof of Residency (Provide One)
- Residential Lease- parents & student(s) names included on lease
- Deed of Property or Mortgage Documents
- Recent Tax, Light or Water Bill – ONLY IF YOU OWN

Current Immunization/ Vaccination/ Shot Records
- According to Texas Department of Health

Birth Certificate
- Alternate: PASSPORT- for students who come from another country ONLY

Social Security Card (if student has one)

Report Card(most Recent) –withdrawal records

Standardized Test scores (most recent)
- TAKS, STAAR, Etc...

Identification Credentials of Both Parents (legible copies are fine)
- State ID card, Driver’s License or Passport

Custody/ Court Documents (if applies)
Please bring all your copied documents during registration if you were not able to upload them during the online pre- registration
