Please note: students are unable to participate in athletics until physicals and online paperwork is complete. They must be finished before tryouts.
Hello MMS Parents,
Welcome to the Memorial Middle School athletics program. With each new season comes a lot to make sure that everyone gets the information needed to participate in any athletics program. This would be volleyball, football, basketball, track, soccer, and cross country. The following paperwork is required for athletics. We now require all of our athletes to complete the majority of the paperwork online. The actual physical completed by the doctor needs to be printed and handed to a coach. You also need to upload a copy of your insurance card.
